Florida law: Chapter 64E-19

Body piercing salons in Florida are regulated by the Department of Health under Chapter 64E-19 of the Florida Administrative Code. A salon must keep a written, notarized parental-consent record on file for every client under 18 before performing a piercing. This form is the Department of Health’s template for that record. Use of the exact form is voluntary, but salons rely on it to satisfy the record-keeping requirement, so most will ask for it.

Who can give consent

Only the minor child’s natural parent or a legal guardian appointed by a court may sign. The signer swears or affirms, under penalty of perjury, that they have the legal authority to consent to the body piercing of the child. A grandparent, sibling, family friend, or older partner cannot provide consent unless they are the court-appointed legal guardian. Bring identification — the notary must verify the signer’s identity.

What the salon needs on file

The completed form identifies the parent or guardian, the minor child and their date of birth and age, and the specific body location(s) the parent consents to have pierced.
